Additional Fees: 

 

There is an additional $10 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Schools

  • Life Insurance

  • Family Medical Leave

  • Immunizations record printout    Free

  • Vaccine record (hard copy)  $7.50

  • Complete medical record  $25.00

  • Effective March 1, 2018, no show appointments and cancellations with less than a 24hr notice will be charged $25
